a patient is weighedThe George Institute Centre for CardioVascular Health is a specialist vascular clinic with a focus on preventive care. The Centre comprises of specialists and health care professionals working together to provide a comprehensive approach to vascular risk management.

Patients  must be referred by their General Practitioner before making an appointment at the Centre.

Working with GPs

The Centre for CardioVascular Health provides an all-inclusive approach to vascular health management. Importantly, the Centre operates under a collaborative approach with referring GPs, in order to achieve optimum vascular health in their patients. General Practitioners (GPs) receive information on the outcome of their patient’s visit, including any medical and lifestyle treatments prescribed and suggested follow-up.

The Centre includes assignment of an allied health care professional as a Case Manager, who focuses on continuity of care for patients as they are seen by relevant specialists and other health care professionals. Patients with acute problems related to their vascular health requiring urgent care  may be referred to external specialists for further treatment.
