Guiding Patients Through Their Treatment

A patient visit to The George Institute Centre for CardioVascular Health

  1. The GP refers the patient.
  2. Consultation booking made by patient.
  3. At the first consultation, the patient meets their Case Manager, who will remain their primary contact person.
  4. The Case Manager arranges for a basic evaluation in six major areas of cardiovascular health - lipids, blood pressure, nutrition, lifestyle, smoking and physical activity.
  5. This basic evaluation provide a multifaceted analysis of the patient’s situation - often ‘making connections’ that would be missed if a single risk factor approach is adopted.
  6. The Case Manager leads the patient through their consultation with one or more of the Centre’s specialists, in collaboration with the referring GP.
  7. Specialists make treatment recommendations where appropriate.
  8. The referring GP receives a report on their patient’s evaluation and recommended management.
